Evolution Development Academy, Inc. (EDA)

1. Explicit Acknowledgment of Inherent Risk

I, the legal consenting adult signing below (acting either on behalf of myself as the athlete, or as the legal parent or guardian of the minor athlete named below), acknowledge that participation in high-performance speed training, maximum-velocity sprinting, block starts, acceleration testing, and biomechanical drills involves extreme physical exertion and violent mechanical forces. I understand that these activities carry significant inherent risks, including but not limited to severe muscle strains, joint sprains, ligament or tendon ruptures (including acute Achilles tendon failure), cardiovascular stress, heat-related illness, and accidents occurring at the training facility or track surfaces. I explicitly agree that the athlete is participating voluntarily and assuming all inherent structural, physical, and environmental risks.

2. Physical & Medical Fitness Disclosure

I certify that the athlete is in excellent physical health, holds a high capacity for high-intensity athletic conditioning, and has no known medical conditions, underlying illnesses, or cardiovascular defects that would prevent safe participation. I agree to explicitly disclose all previous, chronic, or pre-existing injuries to EDA coaching staff prior to the first training session.

3. Performance Chemicals, SARMs, & Substance Certification

I explicitly certify that the athlete is not currently using, nor has utilized within the past twelve (12) months, any unprescribed performance-enhancing drugs, anabolic agents, or Selective Androgen Receptor Modulators (SARMs)—including but not limited to Ostarine (MK-2866), Ibutamoren (MK-677), Ligandrol (LGD-4033), or Enclomiphene. Because EDA's training parameters and maximum-velocity programming rely entirely on natural biological limits and healthy tendon architecture, EDA is completely absolved of liability for structural injuries (such as tendon ruptures or muscle tears) resulting from hidden, withheld, or falsified medical and chemical data.

4. Release, Waiver, and Indemnity Agreement

In consideration of being permitted to participate in EDA programs, I hereby release, waive, and forever discharge Evolution Development Academy, its directors, officers, employees, independent contractors, coaches, and facility operators from any and all liability, claims, demands, or causes of action arising from personal injury, accident, or illness sustained during training, including any claims alleging ordinary negligence on the part of EDA or its representatives.

Furthermore, I agree to indemnify, defend, and hold harmless Evolution Development Academy, its directors, officers, employees, and agents from any and all financial liabilities, legal fees, court costs, or judgments arising out of any lawsuit or claim brought by, or on behalf of, the minor athlete named below.

5. Emergency Medical Authorization

In the event of a medical emergency or acute injury, I authorize EDA coaches, staff, or directors to secure necessary medical treatment, ambulance transportation, or physician care if I am unavailable. I accept full, unreserved financial responsibility for any medical expenses, transport costs, or facility fees incurred.

6. High-Speed Video Analysis & Laboratory Media Consent

I acknowledge that high-speed video analysis is a fundamental, mandatory cornerstone of EDA’s "Lab" environment for technical biomechanical breakdowns and educational feedback. I grant consent for the athlete to be filmed for these internal training purposes. I agree that such technical footage will not be utilized for public social media, marketing, or promotional campaigns unless a separate, explicit Media Release is signed and agreed to.

7. Digital Acceptance of Terms

By completing this digital registration, purchasing a session, or booking a membership through this website, I confirm that I am the legal parent or guardian of the athlete, and that I have thoroughly read, understood, and voluntarily agreed to the comprehensive terms of this legal waiver.
